Breast augmentation is one of the most common cosmetic surgery procedures in the U.S. and one of the most frequently performed procedures by Dr. Papalian. Breast Augmentation can give women with small or unevenly sized breasts a fuller, better-proportioned look through the placement of saline or silicone implants under the breast or chest wall muscle. Commonly, women who have lost breast volume after childbirth and breastfeeding select Breast Augmentation to replace their pre-pregnancy breast volume, which can create a firmer, natural, more lifted look.

As with all cosmetic procedures, there are multiple choices and options for Breast Augmentation. In particular with augmentation, having a comprehensive consultation with Dr. Papalian and taking advantage of his experience and expertise is paramount to making the right choice for you. While breast augmentation with implants is still the most common technique, Dr. Papalian is one of the few Plastic Surgeons specializing in breast enhancement with Autologous Fat Transfer/ Fat Micro Grafting. Dr. Papalian is often requested to “correct” poorly placed or contracted implants, performed by physicians who are not Board Certified Plastic Surgeons. During your consultation with Dr. Papalian, a discussion will take place regarding your particular anatomy, proportions and desires. The consultation will cover your choice for breast augmentation by implants or fat transfer, implant type (saline vs. silicone), implant size, implant position (sub-muscular vs. sub-glandular), implant approach or incision (endoscopic transaxillary, peri-areolar, or infra-mammary), implant texture (smooth vs. textured) and implant shape (round vs. teardrop). Dr. Papalian performs all of these procedures and tailors every operation to each patient’s individual needs. It is critical to a successful outcome to be sure your surgeon discusses your choices and is able to offer a range of options,  including augmentation by fat transfer/micro grafting, to best meet your needs.

Considering all of your options and having a thoughtful discussion with your surgeon are important to achieving an optimal outcome with Breast Augmentation. For this reason, Dr. Papalian allows for a second consultation or “sizing appointment” when you will have a focused discussion regarding your desired breast size. The appointment will be several weeks before your procedure in order to give you ample time to ensure you are making the right decision for you. Additionally, you will return for a pre-operative visit to finalize your choices and ensure that all of your questions are answered and that you are fully prepared for your procedure.

Breast augmentation by Autologous Fat Transfer/Fat Micro Grafting (performing liposuction then using your own fat to increase breast size) is an option for many women, particularly in patients with volume loss following pregnancy. Dr Papalian is one of the few plastic surgeons in the San Francisco Bay area to offer this exciting technology which can answer the dilemma for women who desire more breast volume, but do not desire implants.

Mastopexy (Breast Lift) in Palo Alto

A woman's breasts may develop “droop” or ptosis as a result of the natural effects of aging and gravity, pregnancy, breastfeeding, or weight loss. A breast lift, also called a mastopexy, is performed in order to return a youthful shape and lift to breasts that have sagged or lost volume and firmness. Breast lifts rejuvenate the breasts by trimming excess skin, lifting the nipple and tightening supporting tissues to achieve a firmer, uplifted contour.

As with all cosmetic procedures, there are multiple options for breast lifting. At The Plastic Surgery Center on Woodside, discussing your concerns with Dr. Papalian will allow you to have all your options presented, not just one.

Dr. Papalian performs both minimal incision mastopexy techniques, as well as the more traditional Wise-anchor pattern mastopexy. Minimal incision techniques, such as the peri-areolar mastopexy and the vertical mastopexy are popular procedures for women with minimal to moderate “droop” or ptosis. Complete correction of ptotic breasts, which have fallen significantly below the inframammary folds, may require the more traditional techniques. Simultaneous breast augmentation is a common addition for women who desire more fullness, and is easily added to the mastopexy procedure. Addressing the size of the areola is also a typical component of mastopexy.

Women frequently feel that they have breast ptosis due to the loss of breast volume after pregnancy or weight loss. In conditions where there is loss of breast volume, yet the nipple has not fallen below the inframammary fold, known as pseudo ptosis, mastopexy is usually not required and patients will benefit from Breast Augmentation only.
